Student Grievance Form

A student having a grievance should submit a completed Student Grievance Form to
the Director of Student Services within 30 business days from the date of the action
giving rise to the grievance or within 30 business days of the conclusion of informal
attempts to resolve the situation. Determination as to whether complaints filed after this
deadline will be considered will be done on a case-by-case basis. If the grievance involves the
Director of Student Services, the grievance should be presented to the Office of Human
Resources. Only one subject matter may be included in each grievance. If you need to
make any subsequent changes to the grievance, these changes must be submitted in
writing.

Items marked with an asterisk (*) are required.

A new form must be completed for each grievance.
