Accidental Death and Dismemberment

Your state health coverage includes $5,000 of Basic Group Term Life Insurance with $5,000 of Accidental Death and Dismemberment (AD&D) coverage at no cost to you.

For a monthly premium, additional voluntary AD&D coverage is available to you and your family. This provides financial protection in the event of certain accidental injuries or accidental death. Fort Dearborn Life Insurance Co. administers this insurance plan.

You can choose the amount you want in increments of $5,000 up to $200,000. The benefit pays your beneficiaries the full amount of your coverage upon your death. See these ERS Web pages on the policies to learn about the required premiums.

You also may enroll family members in the plan. If your family member dies, you receive a percentage of the coverage. For more information, see these ERS Web pages.

Apply for coverage within your first 31 days on the job, if you have a qualifying life event (QLE), such as a marriage or birth, or during summer enrollment. Evidence of insurability is never required for this benefit.
