Your state health coverage includes $5,000 of Basic Group Term Life Insurance with $5,000 of Accidental Death and Dismemberment (AD&D) coverage at no cost to you.

During your first 31 days of employment, you can get Optional Term Life Insurance Election I or II (one or two times your salary) without providing evidence of insurability (EOI). You must provide EOI at any time if you enroll in Optional Term Life Election III or IV (three or four times your annual salary). Each election provides an equal amount of AD&D coverage.

Your monthly premiums and the amount your beneficiary will receive depend on your age, salary and the type of policy you buy. Optional term life insurance is limited to a maximum of $400,000 or four times your annual salary, whichever is less.

You also can apply when you have a qualifying life event (QLE), such as a marriage or birth, or you can apply during summer enrollment. EOI is required, so coverage is not guaranteed.

Dependent Term Life Insurance

You can enroll eligible dependents in term life insurance for $1.45 a month. This includes $5,000 of term life with $5,000 AD&D for each family member.

If you sign up for this coverage within your first 31 days of employment, no EOI is required. You may also apply when you have a QLE or during summer enrollment, but an EOI will be required, so coverage is not guaranteed.

You can add a newborn within 30 days of birth without EOI.
