Transfer Student Veteran Information

Transfer students already receiving VA benefits should report to the ASU Veterans Affairs Office to fill out a Request for Change in Program or Place of Training. This is Form 22-1995 for Chapters 30, 33, 34/30, 1606 and 1607 – or Form 22-5495 for Chapter 35. These forms can also be submitted for a change of major or program.

Students using a Hazlewood Exemption are required to bring a copy of the number of Hazlewood hours they have used or the number they have remaining to the Student Accounts/Bursar’s Office, Room 100 in the Mayer Administration Building.
