Community Development Initiatives collaborated with numerous agencies and community-based organizations over the 12 month period from January to December of 2015 to produce the Community Health Needs Assessment of the Poor & Extremely Poor in West Texas.


The project seeks to identify, prioritize, and report health needs of vulnerable, low-income populations in a 20-county region of West Texas.  We invite organizations and communities to use the resources available on these web pages to help inform strategic planning and collaborative efforts to improve community health.
