Community Health Needs Assessment Comprehensive Report

County Health Profiles

The Community Health Needs Assessment employed a collaborative community-based research approach to evaluate the health status of vulnerable and underserved populations in the 20-county study region.

The resulting County Health Profile reports include:

  • A demographic profile of the community
  • A health status profile of health resources, utilization patterns, and rates of morbidity and mortality
  • The sub-regional results of the Survey of the Poor & Extremely Poor in West Texas
Community Development Initiatives originally produced the County Health Profiles as part of the Community Health Needs Assessment of the Poor & Extremely Poor in West Texas.
