Survey Requests

SurveyMonkey is the campus-approved system for surveying faculty, students, staff and alumni. It can also be used to survey the community for research purposes. Survey design and implementation can be done easily by completing and submitting this SurveyMonkey Request Form. If you have any questions, please contact e-Learning at (325) 942-2334, ext. 6263.

Getting Started

  1. Begin by filling out the SurveyMonkey Request Form.
  2. E-mail your completed form and any of your questions to elearning@angelo.edu.

Items to Consider

  1. Please allow one week to process your survey request.
  2. If you are going to distribute the survey via e-mail, make sure to include an e-mail message for recipients.

Getting Results

How will you use your results? SurveyMonkey has several reporting methods that you can choose from:

  1. Microsoft Excel for use in statistical analysis software
  2. Summary report
  3. Charts/graphs for individual questions

Closing Survey

Decide what you would like to do with the survey when you are through collecting results:

  1. Reuse it for next semester/year.
  2. Remove the survey from the system because you never plan to use it again.
