Free Application for Federal Student Aid (FAFSA) Priority Deadline: April 1.
Submit online the FAFSA or Renewal FAFSA to apply for federal and state grants, loans, need-based scholarships and work-study. Include ASU’s Federal School Code (#003541) in the “School” section to ensure the Financial Aid Office’s electronic access to the Student Aid Report (SAR). Allow about two weeks for electronic submission and processing. Additional documents may be required if your FAFSA information (SAR) is selected for verification or to clarify your individual circumstance. You will be notified if additional documents are required.

Institutional Scholarship Application

This application is used to apply for the Carr Academic Scholarship, Transfer Scholarships, Departmental Scholarships and General Scholarships.

Summer Supplemental Application Priority Deadline: March 1.
Submit this online form to apply for financial aid for Summer I and Summer II terms (excluding the Carr Academic Scholarship). Students submitting the Summer Supplemental Application Form online are strongly encouraged to verify the form was submitted and received by checking RAMS or RamPort. After entering your Campus ID and PIN, click on “Financial Aid,” then click on “Eligibility” and go to “Student Requirements.” Please allow two business days for the forms to be posted in RAMS and RamPort.

Verification Worksheet Students who have been chosen for verification by the federal processor use this form when their Free Application for Federal Student Aid (FAFSA) is processed. (Do not submit this form unless the Financial Aid Office requests it.) VW10  
Statement of Income and Expenses Due to the minimal amount of income reported on the financial aid application, this form may be required for additional clarification of the student’s or parent’s situation. (Do not submit this form unless the Financial Aid Office requests it.) IE10  
Federal Parent Loan (PLUS) Borrower Authorization Form After a student receives the award notification and enters online acceptance for the PLUS award, the parent borrower is required to complete this form and submit it to the Financial Aid Office. PLUS
Application for Appeal – Satisfactory Academic Progress This form is available for students with special circumstances that prevented them from meeting satisfactory academic progress requirements. SAPAPP
Emergency Loan Application and Promissory Note This form is used to apply for short-term university loans in anticipation of financial aid awards, or for students who need help paying for tuition and fees and are not applying for financial aid. These loans are due and payable within the semester they are borrowed. EMER
Dependency Change Request Students who do not meet the definition of an independent student as outlined in the Free Application for Federal Student Aid (FAFSA) may use this form if they would like to apply for a dependency change due to an unusual circumstance. DCR10  
Dependency Change Request - Renewal Students who have been approved for a dependency change in the previous year may use this form if they would like to renew the request for the next application period. DCRR10  
Special Circumstance Application This form is used to document a significant change for the worse in the financial situation of the student, the student’s spouse and/or the student’s parent(s). Changes may include a loss of job or benefits, extreme medical expenses, etc. SCF10  
Perkins Loan Master Promissory Note This form is used to finalize your awarded Perkins Loan. Funds will not be disbursed until the completed promissory note is returned to the Financial Aid Office. Faxes cannot be accepted as we must have the original signed document. Please do not complete this promissory note unless you have been awarded a Perkins Loan. PERK
Hinson-Hazlewood College Student Loan Program (Exit Interview) Students who borrow funds from the College Access Loan (CAL) program of the Texas Higher Education Coordinating Board and drop below half time or graduate will need to complete the Exit Interview. Completed forms must be returned to the Financial Aid Office. (Do not submit this form unless the Financial Aid Office requests it.) CALEXIT
