Health Risk Assessments

A Health Risk Assessment (HRA) is an important wellness tool that helps an individual to determine his/her level of risk based a number of factors. HRA’s are questionnaires that, when completed, give you a health risk score to let you know where you are in terms of general health risks. HRA’s are not a replacement for professional medical advice, but are useful in providing motivation for making changes in health and lifestyle. Faculty, staff and students within the ASU community have multiple HRA options available, as listed below. We encourage you to take one or more of these assessments to give you a sense of what your personal risk factors are.

Testwell.org (for everyone)

Testwell offers a free online Holistic Lifestyle Questionnaire that can be used as an educational and awareness tool. Based on the six-dimensional model of wellness as conceived by Dr. Bill Hettler, this multi-dimensional model emphasizes the importance of creating balance in one's life. There are four assessments available (teen, college, adult and older adult).

Kinesiology HRA research project (faculty and staff only)

During the 2010 spring semester, all full-time ASU employees were asked to complete the online AMESmyfile health risk assessment (HRA). To obtain a representative sample with a high confidence level, the goal is to have 200 employees complete the HRA. Upon completing their HRA's, employees received an individualized personal health record (PHR) and health risk assessment report.
