Transcript Requests
You may order your official transcript from Angelo State University by completing and printing the Transcript Order Form.
Transcript Request FormPlease note:
- A transcript is a comprehensive record of your academic work. Since transcripts contain confidential information, they are only released in accordance with the Family Educational Rights and Privacy Act of 1974.
- Each transcript request must contain the student’s original signature and date of request.
- Cost: No cost for first 20 transcripts requested each academic year. Beyond this limit, there is a $3.00 fee for each additional transcript.
- Transcripts are usually processed within one business day of receipt of request. During peak periods (such as registration or graduation), please allow additional time.
- Please note that transcripts will not be sent express mail or courier service. Students who wish to have their transcripts sent in this manner may pick up the sealed copy and make their own arrangement for this type of service.
- If there is a hold on your record, which prevents you from receiving a transcript, you will be contacted by phone or in writing. The notification will include the contact information of the office which placed the hold. You must contact the office listed to have the hold removed. Once the hold has been removed, you must notify the Registrar’s office that hold has been released. At that time, the transcript request can be processed.

To request a transcript by fax, you must provide the information above to fax.
Please note:
At the end of term, once grading begins for each semester and summer session, transcripts for currently enrolled and graduating students will not be available due to the input of grades and posting of degrees for students. Students may view their grades using RAMPORT or the RAMS system. Students may continue to submit requests for transcripts, and the requests will be processed when end of term processes are completed during the week following final exams and graduation.
Instructions for Completing the Transcript Request Form
- Print the form
- Fill in your full name and current mailing address in the space available at the top of the form.
- In the box labeled “FOR COMPLETION BY STUDENT” fill in the blanks to indicate the number of copies desired, if you wish for the Registrar’s Office to hold the transcript, and the purpose of the transcript.
- In the space at the bottom-left of the form, indicate the address to which you want the transcript sent.
- In the space at the bottom-right of the form, complete the information requested (SSN, name, name under which enrolled, date last enrolled, graduation date).
- SIGN AND DATE THE FORM without an original signature and date, the request will not be processed; electronic, or scanned signatures are NOT valid for requesting a transcript.
