Financial Aid Forms

Many forms can securely submitted through our Student Forms portal.

Free Application for Federal Student Aid (FAFSA) Submit online the FAFSA or Renewal FAFSA to apply for federal and state grants, loans, need-based scholarships and Work Study. Include ASU’s Federal School Code (#003541) in the “School” section to ensure the Office of Financial Aid and Scholarships’ electronic access to the Student Aid Report (SAR). Allow about two weeks for electronic submission and processing. Additional documents may be required if your FAFSA information (SAR) is selected for verification, or to clarify your individual circumstance. Texas Application for State Financial Aid (TASFA)

House Bill 1403/Senate Bill 1528 state law allows non-citizen students to pay the resident tuition rate while attending public institutions of higher education in Texas, and be classified as state residents for tuition purposes. As Texas residents, such students are eligible to apply for some student financial aid programs offered by the State of Texas.

Dependent Verification Worksheet

Students who have been chosen for verification by the federal processor use this form when their FAFSA is processed. (Do not submit this form unless the Office of Financial Aid and Scholarships requests it.)  DVW 23-24 DVW 24-25

Budget Change Request Use this form to request budget adjustments for education-related expenses or for expenses that might have impacted your regular expenditures. BCR 23-24 BCR 24-25
Dependency Change Request Students who do not meet the definition of an independent student as outlined in the Free Application for Federal Student Aid (FAFSA) may use this form if they would like to apply for a dependency change due to an unusual circumstance. Also, students who have been approved for a dependency change in the previous year may use this form if they would like to renew the request for the next application period. Special Circumstance Application This form is used to document a significant change for the worse in the financial situation of the student, the student’s spouse and/or the student’s parent(s). Changes may include a loss of job or benefits, extreme medical expenses, etc. SCF 23-24 SCF 24-25

Appeal for Standards of Academic Progress (SAP Appeal)

Students who are not meeting financial aid standards of academic progress use this form to submit an appeal to be considered to have aid restored.  Submit the completed form, a personal statement regarding the appeal, and any supporting documents.
