Student Absence Notification Request Form

The Absence Notification Request is to serve as a resource for students who need to have their professors notified of their absences due to an illness or extenuating circumstances. An absence notification is submitted to the professor by the Office of the Dean of Students as a courtesy but does not excuse students from classes and/or course requirements. The instructor will determine based on their course attendance policy if the absence(s) are excused. This policy applies to cases where the student will require an absence from class for more than one week or special assistance is needed.

Responsibility for class attendance rests with the student. Regular and punctual attendance at all scheduled classes is expected, and the University reserves the right to deal at any time with individual cases of non-attendance.
