To travel abroad, students need a passport. Students who are provisionally accepted into a study abroad program and do not hold a passport need to apply for one as soon as they receive their provisional acceptance letter/email.
Every student participating in a summer study abroad program has to provide the Center for International Studies with a photocopy of the relevant passport pages by March 1.
Emergency Medical Treatment Form
All provisionally accepted study abroad students have to sign an Emergency Medical Treatment and bring the signed hard copy to the Center for International Studies.
Health Clearance Form
The ASU Center for International Studies Program (CISP) Health Clearance is a non-waiveable requirement for participation. All students must comply with the health clearance requirement before departure and within stipulated deadlines.
Students who are not in compliance may face dismissal from their selected study abroad program. You should give the Confidential Health History Form to a physician/nurse prior to your examination.
Students may have the Health Clearance Form completed by consulting their personal physician or by contacting the ASU Health Clinic.
We are pleased to announce that Angelo State University launched a new comprehensive travel risk management program partnered with On Call International to provide you emergency and non-emergency services and benefits. The Global Assistance Program will provide services ranging from pre-trip planning, to telephone advice, to full-scale medical and security evacuations.

All students, employee leaders, and dependents (while traveling with employee leaders) are covered by this plan when outside their home country on Angelo State University related travel or assignment.
