The following template may be used to create your curriculum vitae:
Converted to PDF
House Bill 2504 requires all CVs to be in PDF format. To make your documents accessible, follow these steps:
- Open your document in Microsoft Word
- Click “File,” located on the upper left-hand corner
- Click “Export”
- Click “Create PDF/XPS” and save to a desired location
How to Upload your CV
ASU requires that your vita be in Adobe PDF format to upload in Blackboard. See our managing your CV knowledge base article for information on converting a Word document to a PDF and steps to upload your CV in Blackboard.
Make it Accessible
Make sure your CV is accessible before loading into Blackboard. This means all headings must be tagged, as well as any images or tables. If you have an image or table, make sure they are captioned properly, as well.
Concourse is an online syllabus system that provides a common organization of course information. Concourse is integrated within Blackboard for easy access and helps maintain consistent, up-to-date, accessible syllabi.
Syllabus Statements
Every syllabus at ASU is required to include certain statements per our university O.P. All required statements have already been included in your Concourse template for you, and you can also view these on our Syllabus Statements website.
University O.P. Regulations
The university’s O.P. has several regulations for a syllabus. Make sure to read these sections of the O.P. before uploading your syllabus to your courses:
Faculty Responsibilities and Duties: OP 06.14 (Sections 1, e & Section 1, f, 1)

“The faculty member has a responsibility to strive to maintain those skills and values that ensure the continuation of free and open inquiry. Specifically, within the first week of the semester, faculty members must provide access to a course syllabus that includes:
- The course outline, including each major assignment and examination
- Any required or recommended reading
- The expected learning outcomes form the course
- The methods of assessing those outcomes
- The criteria for grade determination
- The date and time of the final examination (See OP 10.10)
- Office hours and/or hours of outside-of-class contact”

“Additionally, the faculty member should:
- Include the course syllabus statements on university policy concerning academic honesty (University Honor Code), reasonable accommodations for students with disabilities (OP 10.15) and student absence for observance of a religious holy day (OP 10.19)”

Faculty members are required to insert the following statement into each course syllabus:
Any student who, because of a disability, may require special arrangements in order to meet the course requirements should contact the instructor as soon as possible to make any necessary arrangements. Students should present appropriate verification from Student Disability Services during the instructor’s office hours. Please note: instructors are not allowed to provide classroom accommodations to a student until appropriate verification from Student Disability Services has been provided. For additional information, please contact Student Disability Services in the Houston Harte University Center, Office Suite 112, or call 325-942-2047.

- “Faculty are responsible for informing students about technical skill prerequisites and required hardware, software, and supplemental materials necessary for course participation in the catalog description or syllabus.”

How to Create a Concourse Syllabus
Syllabus sections may have ‘Comments’ for the instructor, shown in the yellow box under each heading. Comments will alert you to important information regarding that part of the syllabus. Tips for editing that section, as well as required policies for ASU classes, may be included. Please read the comments carefully. Comments in a Concourse syllabus are only viewable to the editor in the syllabus edit mode, not to students.

Instructors must use their ASU email address (@angelo.edu) in the “Contact Entry”, but may include a high school issued email address in the notes of this area. To add your contact information, select the green “Contact Entry” button. Your high school email can be added in the notes are by selecting the yellow pencil icon.
