Welcome to the ASU employee community. As a newly hired employee, there are a few steps that must be completed before you can begin working.

Student/Temporary Employee Forms
These forms, along with any corresponding work eligibility documentation, should be completed within 3 days from their start date.
The Employee Acknowledgement and Certification form requires that you read and acknowledge the following disclosures and operating policies (OP):

Please download, complete (per instructions), and sign the following forms:
- Employee Information Sheet. Complete entire form.
- Form I-9 - Employment Eligibility Verification. Complete up to the Employee’s Signature and Date line. (printable I-9)
- W-4 (2024) form. Complete the first page titled Employee’s Withholding Certificate. (printable W-4)
- Direct Deposit form. Complete Sections 1-4, Sign Section 5. (printable direct deposit form)
- Employee Acknowledgement and Certification form. Initial and sign form. (Click here if issue downloading form.)
- Voluntary Self-Identification of Protected Veteran Status form.
- Voluntary Self-Identification of Disability form.
These forms, along with any corresponding work eligibility documentation, should be brought to the Office of Human Resources within 3 days from their start date
