New Employees

Please download, complete (per instructions), and sign the following forms:

1. Employee Information Sheet. Complete entire form.
2. Form I-9 - Employment Eligibility Verification. Complete up to the Employee’s Signature and Date line. (printable I-9)
3. W-4 (2023) form. Complete the first page titled Employee’s Withholding Certificate. (printable W-4)
4. Direct Deposit form. Complete Sections 1-4, Sign Section 5. (printable direct deposit form)
5. Employee Acknowledgement and Certification form. Initial and sign form. (Click here if issue downloading form.)
6. Voluntary Self-Identification of Protected Veteran Status form.
7. Voluntary Self-Identification of Disability form.

These forms, along with any corresponding work eligibility documentation, should be brought to the Office of Human Resources within 3 days from their start date
