Community Development Initiatives collaborated with numerous agencies and community-based organizations over the 12 month period from January to December of 2015 to produce the Community Health Needs Assessment of the Poor & Extremely Poor in West Texas.

The project seeks to identify, prioritize, and report health needs of vulnerable, low-income populations in a 20-county region of West Texas.  We invite organizations and communities to use the resources available on these web pages to help inform strategic planning and collaborative efforts to improve community health.

These resources (accessible via the navigation links at the top of the page) are provided to our supporters and partners; community leaders and stakeholders; and to all organizations and citizens working to improve community health. The available resources include:

Comprehensive Report & County Health Profiles

Downloadable evaluations of the health status of vulnerable populations in the 20-county region
