Dual Credit Lab Safety Training Confirmation

All ASU dual credit instructors must fill out this form indicating that all of their students have successfully completed lab safety training each semester. Documentation must also be submitted and can include copies of power point presentations, quiz questions, links to training videos, etc. If an instructor teaches different courses (i.e. BIOL 1308/1108 and BIOL 2323/2123), this form must be submitted for each course taught.

You will be required to upload a roster of your students who completed the training. Instructions on printing/viewing your roster in RamPort can be found in the Dual Credit Instructor Handbook.
